When we receive a request from an individual to notify the purpose of use, disclose, correct, add, delete, stop use, erase, or stop provision to third parties of personal information to be disclosed held by us, we will promptly respond to the request after confirming the individual&#8217;s identity. When a &#8220;Request for Disclosure etc.&#8221; is made by a representative<br \/>\nIf the person making the &#8220;Request for Disclosure etc.&#8221; is a representative, in addition to the documents listed in (1) of 2., please enclose a copy of one of the documents listed in (1) of 3. below that certifies the representative&#8217;s identity, and (2) of 3. a copy of a document certifying the representative&#8217;s identity.<br \/>\n1) Request form prescribed by the company<br \/>\nIn the case of a representative authorized by the individual to make a Request for Disclosure etc.<\/p>\n<ul>\n<li>Letter of attorney from the individual (original)<\/li>\n<\/ul>\n<p>If the representative is a legal representative of a minor<\/p>\n<ul>\n<li>\u2460Certificate of registered matters regarding guardianship registration etc.<\/li>\n<li>\u2461Other official documents that can confirm the authority of legal representation\n<\/li>\n<\/ul>\n<p>2) A copy of a document certifying the representative&#8217;s identity<\/p>\n<ul>\n<li>\u2460Driver&#8217;s license<\/li>\n<li>\u2461Passport<\/li>\n<li>\u2462Health insurance card<\/li>\n<li>\u2463Certificate of residence<\/li>\n<li>\u2464Basic Resident Register card<\/li>\n<\/ul>\n<p>\u203bPlease black out or otherwise process information regarding the place of residence after the prefecture.<\/p>\n<p>4. Fees for &#8220;Requests for Disclosure etc.&#8221; and method of collection<br \/>\nOnly in the case of a request for notification or disclosure of the purpose of use, the following fee will be charged per request. Please enclose postage stamps for the following amount when mailing the request form.<br \/>\nFee amount: 800 yen<\/p>\n<p>5. Method of Response to &#8220;Request for Disclosure etc.&#8221;<br \/>\nIn general, we will respond in writing (by sealed envelope) to the address of the individual indicated on the request form.<br \/>\nPersonal information obtained in response to a &#8220;Request for Disclosure etc.&#8221; will be handled only to the extent necessary to respond to the request for disclosure etc.<br \/>\nIn the following cases, we may not be able to respond to a &#8220;Request for Disclosure etc.&#8221;. In such cases, we will notify you to that effect and the reason. Please be advised that a fee will also be charged for non-disclosure.<\/p>\n<ul>\n<li>\u2460When the identity of the individual or representative cannot be verified<\/li>\n<li>\u2461When the prescribed application documents are incomplete<\/li>\n<li>\u2462When the request for disclosure does not fall under the category of \u201cpersonal information for disclosure\u201d (*)<\/li>\n<li>\u2463When there is a risk of harm to the life, body, property, or other rights or interests of the individual concerned or a third party<\/li>\n<li>\u2464If there is a risk of significant hindrance to the proper conduct of our business<\/li>\n<li>\u2465When it is in violation of laws and regulations<\/li>\n<\/ul>\n<p>6. Personal Information for disclosure and Purpose of Use<br \/>\nPersonal information to be disclosed is personal information that constitutes a systematic collection of information, and for which we have the authority to respond to all requests for disclosure, correction, addition, or deletion of content, suspension of use, elimination, and suspension of provision to third parties requested by the individual concerned. However, if any of the following (1) through (4) applies, the information does not fall under the category of personal information for disclosure. The purpose of use of personal information for disclosure is the same as in c.
